位於日本大阪府藤井寺市道明寺本堂。為該寺之主尊，雕造於十世紀。據寺傳記載，該像為菅原道真流放於太宰府途中，順道拜訪伯母覺壽尼時所留下的自刻觀音像。從髻頂佛面至足下蓮台由檜木整材雕出，像不刳空，眼瞳鑲嵌黑石。檜木株幹端直，質地細密，全面不施彩色，以素木潤飾而成，為檀像雕刻之傑作。一九五二年指定為日本國寶。
觀音戴寶冠，冠旁繒帶垂落雙肩，頭上正中植入化佛立像，頂上佛面及左右菩薩面，共有十面。面相豐腴，身軀健碩，上身肌理分明，腰身內收，具有量感。左手持蓮花淨瓶，右手垂下，手指微屈，端立於仰覆蓮台上。身披帔巾，著下裙，腰部繫帶。身戴臂釧、腕釧，胸前飾有項圈，腰腹至膝下垂掛細緻長串瓔珞。衣紋流麗，襞褶翻折交疊，富有變化，刻工細密精巧，表現其柔軟度與卓越之技法。
