位於四川省綿陽市平武縣報恩寺大悲殿。觀音手中持物，據唐（618～907）伽梵達摩譯《千手千眼觀世音菩薩廣大圓滿無礙大悲心陀羅尼經》各有含義。另據《大佛頂如來密因修證了義諸菩薩萬行首楞嚴經》卷六，菩薩因修證圓通無上道，所以能現眾多妙容。
該像正身由整材巨大的楠木雕刻而成。菩薩頭戴寶冠，頂上有三個菩薩頭像。觀音面相豐潤，眉細目秀，端嚴妙好。共一百零四隻手，肩上二手高舉化佛，胸前二手合十，腹前二手施禪定印，其他大手或結印，或舉日、月、寶鏡、寶印等法器，其餘小手呈放射狀的排列成多層同心圓，布列在觀音像身後，形成背光。菩薩披帔巾，著長裙，瓔珞嚴身，腹部微凸，立於仰覆蓮台上。造像雕刻細膩精美，全身描金，金碧輝煌。
