此書係高麗高宗（1214～1259在位）詔令真覺國師為國家師表，讚揚其德行，頒賜大禪師稱號的原文制書，由黃姓書令史書，是研究高麗（918～1392）禪師、大禪師僧科制度的珍貴文獻。一九六二年指定為韓國國寶第43號。
真覺國師法名慧諶，為普照國師弟子，終生致力修禪，宣揚曹溪宗風，是韓國佛教史上重要人物。
此制書寫於七張色綾連結的卷紙上。行筆直截快速，但點畫樣式與楷書相去不遠，這是對公文書寫速度與正確性的要求所致；中國由王羲之書風發展的院體行書，也有著同樣的特色，如《楚金禪師碑》。此卷因年代久遠，保存不甚完好，有些文字已脫落，無法窺視全貌，但仍不失為高麗時期的佳作。
