圖上方為地藏菩薩乘雲而立，手持錫杖，俯視下方地獄，以願力救度亡者。畫面中央雲層上為身披大袍、戴冠執笏的十王與隨從的司錄；下面是乘馬持幡巾引導亡者的行列，幡上書有「統引領亡鬼使者」字樣。整個畫面以地獄為背景，呈現刀山枯木陰森的景象，群像布局有序，情節表現生動。
