柬埔寨戈公省（Koh Kong）出土。十二世紀下半葉巴戎（Bayon）風格塑像。
此像共有十一面，分上下兩層排列，下層七面，上層四面，雙目注視著四方，表情肅穆。每一面皆戴冠，唯上層正面菩薩的寶冠上刻一化佛。
菩薩袒上身，佩戴項圈，繫腰環，著直條紋褶裙，半跏趺坐。兩側各有十一臂，左右對稱，呈扇形分布，均飾臂釧和手鐲，手中持法器，為一雕工細緻充滿動感的青銅造像。
