新疆維吾爾自治區和田地區洛浦縣熱瓦克佛寺遺址出土。原為塔院殘牆上的浮塑。觀音菩薩頭戴化佛冠，寶繒飄垂雙肩。臉形圓潤，前額較寬，雙目微閉，高鼻大嘴，綴耳璫。長頸寬肩，頸佩項圈與蓮紋瓔珞，雙臂飾釧，纖腰深臍豐臀，體型優美。有別於一般所見的圓形頭光，桃形頭光上方尖角內影塑化佛禪坐於蓮花中，圓形邊飾雲紋，內圈有六瓣花卉；前額旁頭光兩側飾卷草紋。菩薩濕衣透體的長裙繫於腰間，以凸起泥條表現衣紋，襞褶流暢，具有印度笈多王朝（約320～550）藝術特徵。
