菩薩頭戴高花冠，寶繒自然垂落至肩。面相豐腴，雙目微闔，嘴角含笑，神態安詳自在。肩圓而寬，形體端莊，腹部微微前挺。左手提握淨瓶，跣足立於束腰仰覆蓮台上。
蓮台下有雙層台基，上層表面框欄八格，內置供養人像；下層為正方形，上刻有發願文「開皇十四年（594）歲次乙卯八月丁丑朔十五日……佛弟子……敬造觀世音像……願亡兒托生四方妙樂國土並為帝王□□及七世所生父母因緣眷屬佛界眾生……」，文字多因風化而難以辨認。
菩薩體型豐滿，裝飾繁麗，胸飾垂承多重，珠串瓔珞垂於胸前交叉穿環。帔帛自肩搭肘轉掛體側，呈內弧形對稱搭至座旁，是隋代（581～618）菩薩裝束的明顯特徵。
