該像藏於日本福岡縣太宰府市觀世音寺寶藏殿。據大正二年（1913）解體整修時發現像內之墨書，得知為真快、明春等數名雕刻家所作，推測約是平安大治年間（1126～1131）之造像。為現存馬頭觀音雕像中唯一超出5公尺高的大型四面八臂造像。一九○四年指定為日本重要文化財。
該像以檜木多材接合而成。每面戴有天冠，上方有化佛坐像。菩薩臉龐圓潤，蹙眉豐頰，瞪目圓睜，張口外露長牙，表現威嚴之姿。正面雙手結印，右邊三手分持斧鉞、劍、念珠，左邊三手為托法輪、執棒及結手印，跣足端立於仰覆蓮台上。下裙衣紋淺雕，襞褶左右對稱，流暢優美。
