菩薩戴高寶冠，繒帶順肩垂至臂側。面相清臞，五官刻劃傳神。著交領廣袖衣，紋理刻劃舒暢。帔帛自兩肩交於腹前，繞肘後外展，呈波曲狀。背光作蓮瓣形，共分三層；外層飾火焰紋；中層僅露瓣尖，飾直線紋；內層外圈飾幾何圖案。頭光呈圓形，外緣飾蓮花紋。此背光及頭光多層重疊的式樣，於北魏（386～534）早期已十分規範，具濃重裝飾趣味。
菩薩跣足立於覆蓮台上，下為四足方台，台框鐫刻發願文「神龜元年（518）四月八日佛弟子盧邊之先為水厄願造觀世音像一區願從今後居眷大小須遇諸佛」。
