原位於山西省長治市長子縣附近寺院。現為日本重要文化財。
菩薩頭戴高花冠，額髮中分。右手施無畏印，左手結與願印，立於覆蓮台上，下為方形高台。帛帶自雙肩垂下於腹前交叉迴繞身側。衣褶採淺雕細刻的手法，將紋理表現妥貼入微，猶如輕紗薄綢。大蓮瓣形背光紋飾繁麗，富有層次感，是北齊（550～577）背光裝飾的一大特色。雕像表面敷彩，背光背面刻千佛。
該像底座四面刻有發願文「……討冠將軍長子縣令魏蠻鉅鹿下曲陽人也……大齊天保三年（552）發次壬申七月丁卯朔十五日辛巳□造石像一軀並千像仰願皇帝陛下太皇太后……」。
