河北省保定市曲陽縣出土。此佛像高肉髻，面相清臞，五官俊秀，神情祥和。右手施無畏印，左手作與願印，立於覆蓮台上。身材修長，著褒衣博帶式袈裟，襞褶刻劃自然流轉，呈多層尖角向兩側飄拂，富裝飾性，頗具厚實感。圓形頭光，內層為蓮瓣形，外層飾蔓草紋樣，周邊有聯珠紋。蓮瓣形背光，外飾火焰紋，細密流暢，內作一圈聯珠紋，體現了北魏（386～534）晚期佛像的藝術特色。
下方的四足方台側面及背面刻發願文「天平三年（536）三月三日定州中山上曲陽縣佛弟子樂龍……等敬造彌勒像一區願天下太平生生世世值佛聞□與一切眾生俱成正覺」，鐫刻清晰。此像與紐約大都會博物館藏牛猷造彌勒佛及脇侍像具有相同的特徵，是秀骨清像、褒衣博帶造像風格的代表作。
