此像係依照西國三十三所觀音寺第四號之巡禮名剎施福寺主尊刻成。觀音下端及左右兩側分別刻「西國第四番」、「泉州卷尾山」、「施福寺」等字。
觀音頭部分三層，最頂層一面，二、三層皆有多面，正中為化佛立像。觀音面相圓滿，著天衣，帔帛繞臂，垂掛前身，立於仰覆蓮台上。正面六隻主臂，分別為當胸合掌、腹前捧鉢及上舉結印，餘臂則從肩後伸出，分持器物。蓮瓣形背光內刻劃多重圈帶，雕飾雲紋、卷草紋樣及種子梵字圓相紋等。須彌座下方的波紋緣自於施福寺中興祖師法海於海上禮拜觀音菩薩情景。
觀音造型勻稱，刻工細緻，顯示匠師純熟之技藝。
