四川省成都市萬佛寺遺址出土。菩薩面相略長而豐潤，細眉高挑，眼細長，鼻梁高直，上唇厚下唇平實，是菩薩面相中特殊的形式。額髮對分，髮紋疏簡，戴高花冠，冠上部透雕忍冬紋，正中為一化佛坐於蓮瓣形龕中，額上及兩側為卷雲山花裝飾。
菩薩略呈俯視狀，神情端莊，形象清秀高雅，屬隋（581～618）唐（618～907）時期風格。製作上強調肌膚的質感表現，打磨細膩，充分提升材質美感。
