位於戒壇院四隅。該堂中央有一座多寶塔，四隅分別為握劍的持國天、持戟的增長天、右手托多寶塔的多聞天及把筆持卷的廣目天，守護壇場四周。一九五二年指定為日本國寶。
天王像眼瞳均鑲嵌黑石，容貌表情生動，神態威武，身穿鎧甲，雙肩有獅首裝飾，腿套護脛，腳踩踏大頭小身邪鬼。僅持國天王戴頭盔，其餘三尊皆束髮髻，髮紋分綹齊整。增長天王與持國天王怒目圓睜，氣勢逼人。多聞天王和廣目天王瞇眼，似凝視遠方，形成動靜鮮明對比。天王腳下的邪鬼姿勢各有不同，皆蜷縮蹲伏在地，動彈不得。
四天王造像手法自然，衣著、盔甲等質感精巧，結構分明，技法獨樹一格。各像部分漆繪、貼金，並塗以帶狀濃淡同色系之彩色，外表尚留存漸層彩色花紋，與該堂執金剛神立像並稱為奈良時代（710～794）塑像之代表。
